زمینه و هدف: اشرشیاکلی یکی از شایع ترین باکتری هایی است که باعث عفونت دستگاه ادراری می شود. این باکتری به علت اکتساب پلاسمیدهایی که کد کننده بتالاکتامازهای وسیع الطیف (ESBL) هستند، به آنتی بیوتیک های بتالاکتام مقاوم شده است. لذا با توجه به عدم مطالعه نسبت به شناسایی ژن (CTX-M (Cefotaxsim-M در این بیمارستان و منطقه، ژن مذکور از نظر فنوتیپی و ژنوتیپی بررسی گردید.

روش بررسی: این مطالعه توصیفی تحلیلی به صورت مقطعی به مدت 6 ماه از تیر ماه لغایت آذرماه سال 1393 از تمام افراد مراجعه کننده به مرکز آموزشی درمانی شهید یحیی نژاد بابل بر روی 1842 نمونه انجام شد و مقاومت باکتری ها نسبت به دیسک های سفوتاکسیم، مروپنم، آمیکاسین، جنتامایسین، آمیکاسین، سیپروفلوکساسین، سفتریاکسون، نیتروفورانتوئین، نالیدیگسیک اسید، به روش دیسک دیفیوژن انجام شد. ایزوله های مقاوم به آنتی بیوتیک های فوق به وسیله تست تأییدی دیسک های ترکیبی سفوتاکسیم-کلاولانیک و سفتازیدیم –کلاولانیک اسید بررسی شدند. سپس در سویه های مولد بتالاکتامازهای وسیع الطیف وجود ژن CTXM با روش Real Time-PCR بررسی شد. نتایج به دست آمده با برنامه نرم افزاریSPSS  نسخه 20 و آزمون آماری کای دو تحلیل شد.

یافته ها: از تعداد کل نمونه بیماران، 84 ایزوله اشرشیاکلی جدا شد. بیشترین حساسیت مربوط به مروپنم (43/96 درصد)، آمپی سیلین سولباکتام (24/95 درصد)، پیپراسیلین تازوباکتام (5/94 درصد)، آمیکاسین (67/91 درصد) و بیشترین مقاومت به ترتیب به نالیدیکسیک اسید (83 درصد)، داکسی سالیکلین (70 درصد)، سیپروفلوکساسین (2/51 درصد) و سفتریاکسون (63/46 درصد) بود. 29 ایزوله (5/34 درصد) مولد بتالاکتامازهای وسیع الطیف بودند. فراوانی ژن بتالاکتاماز CTX-M فنوتیپ مثبت 69 درصد در اشرشیاکلی به دست آمد. بین وجود ژن CTX-M و ESBL رابطه معنی دار آماری مشاهده شد (03/0P<).

نتیجه گیری: با توجه به نتایج مطالعه حاضر، حضور ژن های مولد بتالاکتاماز (CTX-M) در سویه اشرشیا کلی از نمونه های بیمارستان شهید یحیی نژاد بابل، درصد بالایی را به خود اختصاص داده است (69 درصد). لذا کنترل و پایش مصرف آنتی بیوتیک ها و همچنین مطالعات فنوتیپی و ژنوتیپی بیشتری در باکتری های پاتوژن این منطقه لازم به نظر می رسد.
